We understand that tapering can be a difficult and confusing process. In addition to the TaperMate app, we have launched a pharmacist-led telehealth service for Australian patients to support people to create a unique patient-centred tapering plan following the latest evidence-based guidelines. We'll answer your questions, help you to understand the various tapering methods, and advocate to your prescriber to get the right prescriptions and support to start your taper.
This appointment is to create a personalised medication tapering plan following the Maudsley Deprescribing approach (hyperbolic tapering). We will go through a full medication history and develop a plan to suit your specific circumstances. We will answer any questions you have about your medicines and tapering. We will communicate with your prescriber and advocate for you if you need additional prescriptions (e.g. for compounding). Regular follow up appointments are strongly encouraged.
Follow up appointment for those who already have a personalised tapering plan from the TaperMate Clinic. Select the long appointment if you have multiple questions or concerns to discuss, we will alter your taper plan according to your needs.
You may also select this option if you have already started tapering following a plan developed outside of the TaperMate Clinic, but want some personalised advice about your plan.
Follow up appointment for those who already have a personalised tapering plan through the TaperMate Clinic. Select this option for your regular reviews. Select the long follow up appointment if you have multiple questions or concerns (i.e. will take more than 15 minutes).
